DescriptionAbout the jobYour Opportunity:Wetaskiwin’s Home Care team is looking for a compassionate and caring Registered Nurse. In this role you will have the opportunity to make a difference in the lives of people in your community by collaborating in the delivery of holistic, client and family centered care. Working collaboratively as part of a multidisciplinary team, the Registered Nurse Case Manager is involved in the case management of clients, including ongoing assessment, determination of unmet need, and care planning in consultation with clients to achieve client goals utilizing appropriate internal and external resources. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. In addition, the Registered Nurse Case Manager is responsible for the provision of direct nursing care to the clients. The Registered Nurse Case Manager ensures that client care is evidence based and client-centered by establishing goals for service delivery and evaluating outcomes. The successful candidate will need a range of clinical skills (Wound Care, Palliative Care, HPT, IV etc.), good prioritization and assessment skills, strong communication skills, a collaborative approach to working with your interdisciplinary team, and the ability to be flexible and agile in an ever-changing environment. You will also demonstrate compassion and have a strong focus on patient-centered care. As you expand your experience, you will have the opportunity to advance your clinical skills in multiple areas of practice.Description:As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.17/hour - $42.96/hour ($35.87/hour - $43.82/hour effective April 1, 2023). Work tasks and charting is computer based so there are extensive repetitive motion tasks. Must be able to tolerate long periods of sitting or standing as well as repetitive movements. Must be able to participate in hands on care of patients (i.e., transfers, bariatric clients, bending, Reaching, twisting, and walking distances). Must be able to lift and carry 10-15 kg regularly if needed.
