Working collaboratively as part of a multidisciplinary team, the Registered Nurse Case Manager is involved in the case management of clients, including ongoing assessment, determination of unmet need, and care planning in consultation with clients to achieve client goals utilizing appropriate internal and external resources. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. In addition, the Registered Nurse Case Manager is responsible for the provision of direct nursing care to the clients. The Registered Nurse Case Manager ensures that client care is evidence based and client-centered by establishing goals for service delivery and evaluating outcomes. The successful candidate will need a range of clinical skills (Wound Care, Palliative Care, HPT, IV etc.), good prioritization and assessment skills, strong communication skills, a collaborative approach to working with your interdisciplinary team, and the ability to be flexible and agile in an ever-changing environment. You will also demonstrate compassion and have a strong focus on patient-centered care. Work tasks and charting is computer based so there are extensive repetitive motion tasks. Must be able to tolerate long periods of sitting or standing as well as repetitive movements. Must be able to participate in hands on care of patients (i.e., transfers, bariatric clients, bending, Reaching, twisting, and walking distances). Must be able to lift and carry 10-15 kg regularly if needed.
